Does anyone here have problems with ringtones failing randomly?

No problems with Wizard's latest on my DroidX. I've been on boostedassv2 since it was first offered as well with very low voltage settings and have not noticed any issues.

I just did a factory reset on my phone last week - back to Wizard MIUI 5.12 original and only restored the apps I actually use. It was getting a bit sluggish and cluttered, but after almost 5 months without a factory reset, it was time. However, I set it back up nearly identical to the way I had it prior to the factory reset. I didn't back up any of my system settings; electing to change it all manually instead so that nothing bad carried over in the backup process.

You may want to check your sdcard. I've seen that happen when the sdcard started to fail. My wife's did the same thing with no errors popping up, but when I copied everything off the card and tried to reformat it, the card failed hard. It would constantly set itself to read only. Might be a good place to start. At the very least, backing up your card is a good idea even if that isn't the issue.

Actually, I doubt it's the card. I forgot that I moved my ringtones to the phone using RootExplorer. I still have the same problem.

Is it all the time or randomly? And us it default, assigned or a mix of both?

The tone I want it to play is a short MP3. It works randomly/rarely. When it fails, it just plays a standard ring. The strange thing is, I can't find that ring on my phone. I've deleted all other tones in an attempt to find it, but it keeps playing. I used to have problems with SMS notifications as well, but now I use GoSMS as a work around.
