Unless the phone is turned off, I'm pretty sure the alarm will work regardless of the state....just like a phone call.
What do you mean by sleep mode?

Whenever you press the power button the phone goes into 'sleep mode.' I don't own a DroidX yet so my terminology may be a little off. What do you mean by sleep mode?

i only have silent, airplane, and power off
look up alarm clock plus, i just started to use it, and it looks pretty promising
that is just turning the display off and locking the screen. in that case, even the stock alarm should work, but again check this out Alarm Clock PlusV1
The stock alarm works fine when the phone is in sleep mode. The phone turns off the screen if you have no activity for a set amount of time OR if you manually short press the power button.
As someone else mentioned, it's just like a phone call will wake up the phone (display turns on, phone rings and/or vibrates), the alarm does the same thing.
While you should check out alternate apps, the stock alarm that is in 2.2 works fine for me. (I don't remember if I used it in 2.1, I think it's the same).
It has a "get louder" option that starts the sound reasonably quiet, then turns up the volume the longer you ignore it. With a good ringtone (high energy music), it works really well. I have mine set to vibrate also, and the combination would be extremely difficult to ignore.
