Also have you tried a factory reset? If you let a Verizon rep mess with it, who knows what they did... :/

Tried that after SBF, couldn't get out of bootloader before. Didn't work
Is it rooted? Possibly try flashing a ROM? It really seems more hardware related if you have SBF'ed as well.
If I were you, to rule out firmware issues, I would root it (if you want of course) and get CWM recovery going. Flash the .621 kernel/updater - I know you did it via SBF, but that would re-do it via CWM, making sure it's not an SBF issue. Then flash some third-party firmware (any type, doesn't matter much - this is just for proof of concept) re-flashing the .621 kernel/updater will make sure the hardware is communicating with the software properly, and flashing a third-party ROM will make sure that something in /system didn't get messed up in the SBF as well. If the issue remains, its MOST LIKELY a hardware issue.
do your volume rockers still work? have you ever has problems with your buttons before?

No never any button troubles before. That app looks pretty cool too. Wife used my upgrade and got an iPhone so I'm on her Incredible now. Downloaded Swype beta b/c i hate tapping to type and other than adjusting to the smaller screen, I'm happy for now. Her upgrade comes in on Friday so now I'm wrestling with deciding if I want one of the Moto RAZR HD/RAZR Maxx HD, a GS3, or if I wait and see what the Note II looks like and when it will come out on VZW (Sprint release date of 10/25).
Thanks for all the suggestions, if I get bored at somepoint I may try to play around with rooting it a little.
