I know you can add sounds on your SD card, but how can I add sounds to /system/media and have them show up in the list to choose from on the phone.

I am rooted.

I want a custom sound to be available at all times, whether the sd card is mounted or now.

Copy them there using rootexplorer.

I know how to copy the sound into the folder, but just putting them in the folder does not put them in the list of available sounds. I need to know how to get the sounds added to the list of available sounds.

You have to restart the phone. That is all I had to do. Be sure to put them in the correct subfolders (ringtones, notifications, etc.).

When you manually move a file into a system folder, your player and gallery won't re-index those directories until the phone is rebooted. Same with some photo editing apps. You can always unmount and remount SD card for those, but for system (using the on-board memory, not the SD card) files, you have to reboot to reindex.
